Guiding an Improved Dementia Experience (GUIDE) Model

UToledo Health Geriatrics is pleased to announce that we are a provider of the Center for Medicare and Medicaid’s new Guiding an Improved Dementia Experience (GUIDE) Model, a program that empowers the caregivers of patients with memory loss issues such as dementia and Alzheimer's disease. Through GUIDE, caregivers can access resources, training and support while collaborating with dementia-focused community programs to deliver coordinated plans of care.

What does GUIDE strive to do?

GUIDE services include:

Comprehensive services and care plan

Get an assessment to identify your patient’s individual health needs and build a care plan personalized to the services you need.

Caregiver support

Education and support for a relative or non-paid caregiver such as connecting with a care navigator when they need it

GUIDE respite services

Funding for respite services up to an annual cap can be provided when caregivers need a break. Support can come from adult day centers, nursing homes and in-home caregivers

Coordination and support

24/7 access

Patients are free to see any doctor or hospital that accepts Medicare. UToledo Health collaborates with that serve patients who have dementia.
