What is a Shared Medical Appointment?
A Shared Medical Appointment (SMA) is a group approach to better serve your medical needs. Six to eight patients, with conditions similar to yours, will join you in a group setting for 90 minutes to discuss your group’s shared medical concerns, as well as approaches to treatment. Our medical team includes a physician, nurse practitioner, psychologist, nutritionist, diabetes educator, medical assistants and an SMA assistant.
During these appointments, you will discuss health conditions, test and lab results, preventive screenings and vaccines, as well as your treatment plan to meet your goals.
Patients are encouraged to bring a friend or family member for support. However, every effort is made to keep personal information and medical diagnosis private. Those who attend will be required to sign a confidentiality commitment agreement.
It’s important to note that these types of appointments do not require you to change your primary care provider. All care plans are sent to your regular health-care provider for seamless care.
