Pharmacotherapy Clinic
The UTMC Pharmacotherapy Clinic is a pharmacist-led, outpatient clinic specializing in comprehensive medication management for patients with chronic conditions, including high blood pressure, high cholesterol, heart failure and diabetes.
Our residency-trained pharmacists practice under collaborative practice agreements, allowing them to initiate, adjust and monitor medications independently.
Working closely with primary care providers and specialists, our team delivers evidence-based, patient-centered care to improve health outcomes and reduce hospitalizations. We also provide education, medication adherence support, lab monitoring and assistance with access to high-cost medications.
A core focus of the clinic is individualized care management of patients on blood-thinning therapies, including warfarin and direct oral anticoagulants (DOACs).
Our anticoagulation service is recognized as an Anticoagulation Center of Excellence by the Anticoagulation Forum, offering expert care to our patients. Our pharmacists provide ongoing monitoring, dose adjustments and patient education to ensure safety, therapeutic effectiveness and adherence. For patients on DOACs, we offer structured follow-up to monitor kidney function, assess bleeding and clotting risks, and ensure appropriate use based on clinical guidelines and patient-specific factors.

A: The clinic accepts most medical insurance plans and directly bills your insurance
provider.
Q: Do I need a referral to be seen in the clinic?
A: Yes, a referral from your provider is required. Your physician can place a referral
through EPIC or contact our clinic directly.
Q: Should I arrive early for my first appointment?
A: Please arrive 15 minutes early for your first appointment.
Q: What should I bring to my appointment?
A: Please bring a complete medication list and bottles of all medications, including
any over-the-counter products, vitamins, herbals and supplements to every appointment.
Q. How long will my appointment take?
A. It depends on the complexity of your condition and medication regimen. You can
ask the pharmacist for more information when scheduling.
Q: Who will I see during my visit?
A: You will meet with a residency-trained pharmacist who works closely with your provider.
Because UTMC is an academic medical center, we are committed to training future healthcare professionals. Pharmacy students or residents may be involved in your visit under the supervision of our clinical pharmacists.
